The application and reference forms can be downloaded by using the above links or can be requested by emailing info@damianmillsaward.ca. The application form must be completed in Microsoft Word, printed and mailed to the address on the form. Handwritten and email submissions will not be considered. Three references must be submitted together with the application form. The reference forms must be completed by three referees, one of which must be from a non-cricketing perspective. The referee must complete the forms and return it to the applicant in a sealed envelope. The applicant will then submit the three sealed envelopes together with their application to: Damian Mills Junior Cricket Foundation · The deadline for submissions is June 15th of each year. No applications post-marked after this date will be accepted. · Application material will not be returned under any circumstances, nor will it be used for subsequent years. You may photocopy the completed application for your reference. · Written acknowledgement of receipt of your application will be provided by June 30th each year. · You may use additional sheets if more space is needed to provide information. |
